New firm to tackle shortfalls in senior care

Outcomes Health Information Solutions, based in Alpharetta in northern metro Atlanta, has launched a company that aims to address gaps in the medical care of seniors.

SeniorCare will send nurse practitioners into people’s homes to assess the health of individual seniors and send the information to the appropriate insurers and physicians’ office. It will primarily be a service company, unlike its health IT parent company, Outcomes Health.

A private firm established 20 years ago, Outcomes Health pulls data from physician offices and identifies gaps in the care of Medicare Advantage and other patients.

SeniorCare will take this data and help coordinate the care for patients.

“Now, more than ever, it is important for health plans and providers to have access to real-time data analytics to ensure patients are receiving the right care, at the right time, and in the most appropriate care setting,” said Joell Keim, president of SeniorCare, in a statement. That can produce better patient outcomes, more accurate reimbursement rates, reduced costs and higher satisfaction rates, she said.

Added Wanda Kochhar, founder and CEO of Outcomes Health and a SeniorCare board member, ”Our goal is to help improve and streamline population health management and patient care.”
