A veteran died at Charlie Norwood VA Medical Center after being improperly restrained for days, prompting staff discipline and reforms at the medical center. A veteran in his 60s died at Charlie Norwood VA Medical Center in Augusta after being in restraints for nearly three days that likely contributed to the blood clots in his legs that killed him, the Department of Veterans Affairs inspector general said in a report this week.
